VASCULAR COMPLICATIONS OF MULTIPLE HEREDITARY EXOSTOSES - A REPORT OF TWO CASES

Abstract

Osteochondromas are the most common benign tumours of the bone, observed in 1-2percent of population and can be solitary or multiple. When multiple, they represent an autosomal dominant pattern of inheritance. Patients rarely develop vascular complications. So far only 106 cases are reported in the literature. Here we report two cases of vascular complications of osteochondromas - a pseudo aneurysm and an arterial occlusion.CASE 1- A 19 year old boy presented with pseudoaneurysm of left popliteal artery, of size 15 x 15 x 10 cm,one of the largest reported so far, with features of distal leg ischemia.Patient underwent surgery- the offending exostosis excised and diseased artery with pseudo aneurysm excised and continuity restored with reverse saphenous vein interposition graft.CASE 2- A 32 year male , a known case of left lower limb DVT, presented with recent onset of claudication pain of left calf. CTA confirmed occlusion of distal SFA and popliteal artery. surgical excision of the offending exostosis and a femoro popliteal bypass using reverse saphenous vein from the opposite limb was performed.In both cases the benign nature of osteochondromas was confirmed by HPE.
