CLINICO-RADIOLOGICAL ESTI-MATION EFFICIENCY OF TREATMENT OF CHRONIC GRANULATING PERIODONTI-TIS OF THE CHILDREN’S CON-STANT TEETH

Abstract

Treating of chronic granulating periodontitis, we suggest to promote efficiency of treatment and to level the negative moments of temporal root-canals obturation of the constant teeth for children by introduction in the complement of paste ,on the basis of hydroxide of calcium, means of phytogenous - larkspur tincture that will strengthen regeneration properties of the offered paste simultaneously. Well in an experiment on rats we investigated, that tincture of medical larkspur in composition with offered paste shows out antiinflammatory and osteoregenerative effect. Thus the toxic effect of paste was absent, both on local and on general levels. The Aim of the Research. To learn the influence of the offered local treatment of granulate chronic periodontitis of the children's constant teeth on the processes of regeneration of periodontal bone tissue . Materials and methods. Clinical examination on generally accepted methodologies, X-ray research, supervision and treatment, is carried out in 96 persons aged 12-18, that were divided into 2 groups: • I group, in which 49 children were with chronic granulating periodontitis of the constant teeth; • II group, in which 47 children were with acute chronic granulating periodontitis of the constant teeth. Both researched children's groups with granulating periodontitis were divided into 2 subgroups: the main and the control ones. The two main subgroups of the 1 and 2 groups (32 children with chronic granulating periodontitis and 31 children with acute chronic granulating periodontitis of the constant teeth) were treated with the help of newly done local treatment. The control groups (17 children with chronic granulating periodontitis and 16 children with acute chronic granulating periodontitis of the second teeth) were treated with the help of traditional methodology. For estimation efficiency of treatment the following criterions were used: 1. Clinical; a) Absence of subjective feelings; b) Absence of objective clinical signs of in-flammation (tooth movement, edema and hyperemia of mucous membrane of cell sprout). 2 Roentgen logical: a) closing of root apexes, completion of periodont forming ; b) normalization of roentgen logic picture in a per apical area due to renewing the hearth of destruction of bone tissue by formation of bone beams. With the aim of more exact comparison of indexes of accurate sciagraphy on the stages of treatment of chronic granulating periodontitis of the constant teeth and his intensifying we used a quantitative index - periapical index(РАІ) offered by Orstavik D et al., in modification of А.М. Soloviova, that gives an opportunity not only to educe dilution in a bone tissue but also estimate the structure of bone trabecules and bone-cerebral spaces in a periapical area and simultaneously to take into account the wide clearance of the apex opening root-canal of the children's constant teeth . The method of local treatment of patients in basic sub-groups , worked out by us, envisaged the use of phytogenous preparations and meant antiseptic treatment of root-canals of the second teeth for children by 1% chlorofilipt solution, post apical therapy with larkspur tincture under a temporal bandage(for not more than twenty-four hours) with the aim of anti-inflammatory, necrotic, antihemorragic, immune stimulating and oste-oregenerative actions. Farther, after the removal of the acute inflamation in case of intensifying chronic granulate periodontitis(mostly through 2-3 visits) and in the cases when patients' teeth did not disturb under an impermeable bandage and it was not marked intensifying chronic granulate periodontitis(mostly through 1-2 visits), the temporal multiple filling of root-canal was conducted by anti-inflammatory paste(composition of paste : 1,0 gr of larkspur tincture, 0,5 gr of glycerin, to 3,0 gr of Са hydroxide).
