PECULIARITIES OF CHANGE OPTICAL PROPERTIES OF THE PLASMA OF VENOUS BLOOD IN VARIOUS FORMS OF ACUTE PANCREATITIS

Abstract

Background and Aims: The particulierities of changes in plasma optical density of venous blood in various forms of acute pancreatitis have been studied. Materials and Methods: materials include 197 individuals: 30 healthy donors; 18 patients with non-destructive forms of acute appendicitis and 17 patients with acute cholecystitis; 80 patients with acute edematous pancreatitis; 39 patients with necrotizing pancreatitis. All patients underwent complete laboratory and instrumental examination according to the protocol of care for patients with acute pancreatitis. In addition, all people were determined the optical density of plasma. Results: It was established that in determining the optical density of plasma of venous blood in patients with various forms of acute pancreatitis is the most sensitive of change observed at a wavelength l = 280 nm. In patients with acute edematous pancreatitis a significant increase of plasma optical density was greater than 0.63 units. Development of advanced pancreatic necrosis is characterized by a significant reduction in optical density of plasma venous less than 0.49 units. The results of the first phase of research formed the basis for working out a new way of diagnosing acute edematous pancreatitis, the sence of which is the AGP evaluation by determining the absorption spectra in the wavelength l = 280 nm. Thus, the growth of optical density is greater than 0.63 units, is rated as one of the typical laboratory criteria of edematous forms AP (patent for utility model β„– 62667 UA). In determining the information content of the proposed method of diagnosis found that its diagnostic sensitivity is 84.6%, the diagnostic specificity - 71.4%, diagnostic accuracy - 81.8%, diagnostic efficiency - 78.0%. Results of the second phase of research formed the basis for working out a new way of diagnosing pancreatic necrosis, the essence of which is that in patients with AP determined the optical density plasma of peripheral veins through the study of the absorption spectra in the wavelength l = 280 nm and with a decrease in optical density below than 0.49 units diagnose common pancreatic necrosis (patent for utility model β„– 62380 UA). Accumulated informative method of diagnosis proved that its diagnostic sensitivity is 87.9%, the diagnostic specificity - 73.1%, diagnostic accuracy - 85.7%, the diagnostic efficiency - 77.8%. Conclusion: Spectro-photometric diagnostic methods worked out various forms of highly informative diagnostic of acute pancreatitis, ease of implementation and low cost, which proves their suitability for widespread clinical use.
