A RARE CASE OF AMYANDS HERNIA

Abstract
The presence of an appendix within a hernia sac is known as Amyands hernia. Its incidence is extremely rare 1 of all hernias. The presence of appendicitis among those is even rare and is estimated between 0.07 and 0.13(1). It is often difficult to diagnose it pre operatively it poses even more confusion intra operatively whether or not to perform appendectomy and the type of hernia repair. Case study We had a case of right sided obstructed inguinal hernia for past 2 days with complaints of pain, fever vomiting. We operated it as a case of right sided obstructed inguinal hernia going in for strangulation. To our surprise the hernia content was 8 cm long, turgid inflamed appendix. We did an appendectomy and modified bassinis hernia repair. Patient did well post operatively and discharged on 8th POD. Pathology confirmed it as an acute appendicitis. Conclusion : This case is presented because of its extremely rare occurrence (0.07) and to clarify the dilemma on appropriate management of this rare presentation during emergency surgery.
