Abstract
Breast cancer has become the common cancer in women. Surgery is an important component of multidisciplinary treatment. Breast conservation surgery, Modified Radical Mastectomy and rarely Radical Mastectomy are the operative surgical types followed. Since most patients will require systemic therapy the wound morbidity must be minimum so that systemic therapy can be started at the earliest. Skin incision accounts to the wound morbidity. Here two patients for whom wound morbidity delayed the systemic treatment are described and four patients for whom the modified incision (pyramidal Extension) for peripherially situated lesions has helped in healing the wound faster and cosmetically better
