APPLICATION OF CAT-TEST IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE AND CONCOMITANT ALCOHOLISM

Abstract

Aim. To study the aspects of subjective symptoms perception in patients with chronic obstructive pulmonary disease (COPD) and concomitant alcoholism. Materials and Methods. 147 Men with diagnosed with COPD aged 40 to 88 years were examined. Spirometry, distribution of patients by stages of alcoholism, assessment of COPD impact on the patients life according to the CAT-test, assessment of COPD severity and prognosis of mortality by the BODE scale were carried out. The compliance level was assessed in the Morisky-Green test. Statistical processing was performed using the licensed program SPSS 17. Results. The prevalence of individuals with alcohol dependence was revealed in the group with a relatively positive prognosis for mortality from COPD. A tendency to a more positive perception of COPD symptoms was obtained in individuals with II and III stages of alcoholism with a relatively positive prognosis for mortality from COPD (up to 4 points on the BODE scale) with some worsening of somatic status at the same time. Increase in the risk of mortality from COPD was associated with increase in CAT scores, in patients both with and without alcohol dependence. Alcoholism was associated with the decrease in compliance in all groups of patients. Conclusions. Patients with a relatively low risk for mortality from COPD and concomitant alcoholism showed a tendency to underestimate the impact of bronchial obstruction symptoms on everyday life. Subjective assessment of severe COPD symptoms by patients without alcoholism, and by those who suffered from alcohol dependence, did not differ. The CAT-test score increased with worsening of prognosis for mortality from COPD regardless of the presence of alcoholism, which proved the high efficiency of the questionnaire in both groups. Alcoholism was associated with a decrease in compliance in patients with COPD.
