The State of Epidemiological Situation on HIV Infection / AIDS in Khmelnytskyi Region during 2009-2016

Abstract
During the period from 1996 to 2017, 4491 cases of HIV infection were registered among Khmelnytskyi region inhabitants. The estimated number of people living with HIV (hereinafter referred to as PLHIV) is 3,500 people in Khmelnytskyi region (1.3 million of population). On January 1, 2017 only 1985 PLHIV were under medical supervision including 668 AIDS patients. From the total number of HIV-infected people in Khmelnytskyi region there are 156 children, 50 of whom are diagnosed as HIV-positive, including 47 children with established diagnosis of AIDS. In this region sexual transmission of HIV to common people is more active than environment HIV as it is determined by the combined effect of the epidemic parenteral and sexual pathways of pathogen transmission. In 2016 the number of HIV-positive people infected via sex is 70% in Ukraine, compared with 10% of infected by injecting drug users. In the age structure of HIV-infected people there is a tendency towards an increase in the proportion of people of reproductive and working age. This tendency is maintained since 2009. During 2009-2013, the incidence of HIV infection in Khmelnytskyi region grew with a subsequent decline (2016 – 15.6 per 100 thousand people). The proportion of people who were registered in the III-IV clinical stages of HIV infection, from the total number of HIV-diagnosed people for the first time in life, increased from 39.3% in 2013 to 51.0% in 2016. The result of the HIV-positive people late coming to hospital is an increase in the rates of AIDS and AIDS mortality in the region. During 2009-2016, the incidence of AIDS in the region increased (in 2009 it was 5.4, in 2016 – 10.7), with the highest indicator in 2012 (13.2 per 100 thousand people). The level of mortality from AIDS varies from 4.0 (2009) to 4.4 per 100 thousand people (2016), with a maximum value in 2011 (5.5). The most common AIDS-indicator in the region is tuberculosis. According to statistics in 2016, TB was found among 55.1% of the first registered AIDS patients (2009 – 47.2%, in 2011 – 73.6%). The share of newly diagnosed TB among new AIDS cases remains high and in 2016 it was 52.2% as contrasted to 22.2% in 2009. Also, TB remains the main cause of death among AIDS patients. During 2009-2016, in each of the 1,6 cases of AIDS deaths, the cause was TB (269 cases, 60.9%).
