Experiencia en el diagnóstico del linfoma de Hodgkin con tuberculosis en el Hospital Pediátrico “Dr. Silvestre Frenk Freund”, Ciudad de México

Abstract

RESUMEN Introducción: La presentación clínica de linfoma de Hodgkin (LH) y tuberculosis (TB) con involucro ganglionar origina problemas en el diagnóstico diferencial para el patólogo que examina un ganglio linfático con necrosis o con fibrosis intensa que impida observar las células neoplásicas. El uso de inmunohistoquímica resulta de gran utilidad cuando se presenta esta asociación. El objetivo de este estudio es mostrar la experiencia del servicio de Patología de un hospital pediátrico mexicano en el diagnóstico de LH con TB en población infantil y el uso de inmunohisto-química, tinciones y reacción en cadena de polimerasa (PCR) en este material. Metodología: Se realizó un estudio retrospectivo en donde se evaluaron casos de LH del año 2012 al 2016 en el archivo del Hospital de Pediatría “Dr. Silvestre Frenk Freund” del Centro Médico Nacional Siglo XXI del Instituto Mexicano del Seguro Social (Ciudad de México). Se categorizaron dichos casos de acuerdo a su tipo histopatológico y con/sin presencia de TB. Resultados: Se encontraron 37 casos de LH en total, de los cuales 22 se incluyeron en este estudio; 3 presentaron TB sin encontrarse predilección por algún subtipo de LH. En los tres casos con TB fue necesario el uso de inmunohistoquímica para establecer los diagnósticos. Conclusiones: La experiencia que se tiene en el Hospital “Dr. Silvestre Frenk Freund” en la Ciudad de México demuestra la oportunidad de dar un diagnóstico oportuno y certero en asociación de LH y TB con la utilización de estudios de inmunohistoquímica, además de tinciones para micobacterias y PCR. ABSTRACT Introduction: The clinical presentation of Hodkin's lymphoma (HL) and tuberculosis (TB) with nodal involvement causes problems in the diffe-rential diagnosis for the pathologist who examines a lymph node with necrosis or intense fibrosis that prevents the observation of neoplastic cells. The use of immunohistochemistry is very useful when this associa-tion occurs. The objetive of this study is to show the experience of the Pathology service of a mexican pediatric hospital in the diagnosis of HL with TB in children and the use of immunohistochemistry, staining and polymerase chain reaction (PCR) in this material. Methodology: A retrospective study was performed, cases of HL from 2012 to 2016 were evaluated in the files of the Pediatric Hospital “Dr. Silvestre Frenk Freund” of the Centro Médico Nacional Siglo XXI, Instituto Mexicano del Seguro Social (Mexico City). Those cases were categorized according to their histopathological type and with/without TB. Results: 37 cases of HL were found, of which 22 were included in this study; 3 cases presented TB without finding a predilection for any subtype of HL. In the three cases with TB, the use of immunohistochemis-try was necessary to establish the diagnoses. Conclusions: The experience that we have in the Hospital “Dr. Silvestre Frenk Freund” in Mexico City demostrates the opportunity to give an opportune and certain diagnosis of HL and TB with the use of im-munohistochemical studies, as well as staining for acid-fast bacilli and PCR.
