OPPORTUNITIES OF PREDICTING ADVERSE OUTCOMES IN PATIENTS WITH ARTERIAL HYPERTENSION COMBINED WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Abstract

In patients with arterial hypertension (AH) and chronic obstructive pulmonary disease (COPD), there is an increased risk of strokes, myocardial infarctions and fatal outcomes. In this regard, the allocation of the group with an increased risk in the next 2 years is an urgent problem. 235 patients with hypertension II degree combined with moderate COPD were examined clinically. Spontaneous aggregation of platelet-leukocyte suspension was estimated. Using logit regression analysis the method for predicting the total number of strokes, myocardial infarctions and fatal outcomes was developed within 2 years. The efficiency of the model was tested in the control group of 158 patients with hypertension and COPD. It has been determined that with the development of the total number of strokes, myocardial infarctions and fatal outcomes significantly (p <0.05) correlate with the indices of spirometry in case of FEV-1 less than 70%, spontaneous aggregation of platelet-leukocyte suspension, duration of hypertension and COPD over 10 years as well as the presence of purulent sputum in at least one exacerbation. The sensitivity of the developed method of prognosis was 75%, specificity - 98.6%. High risk of strokes and myocardial infarctions and fatal outcomes in the next 2 years is observed in 5% of patients with hypertension II degree and COPD, it is rational to use antiplatelet agents as a conventional therapy.
