Hemoperitoneum due to bleeding myoma in pregnancy

Abstract

Spontaneous venous rupture overlying an uterine fibroid is a rare cause of hemoperitoneum in fibroid complicating pregnancy.30 years Gravida 2 Abortion1 with history of 5 months amenorrhoea, case of fibroid complicating pregnancy with features of red degeneration of fibroid was transferred from ward in a state of shock. ultrasound revealed haemoperitoneum. There was significant drop in hemoglobin and hematocrit. The patient underwent emergency exploratory laprotomy. There was diffuse bleeding from subserosal venous plexus and massive hemoperitoneum. As bleeding was diffuse, bleeding could not be arrested and proceeded to subtotal hysterectomy. This case suggest that spontaneous intraperitoneal haemorrhage associated with fibroid complicating pregnancy,although rare,should be considered in women with hypovolemic shock and uterine fibroid
