Abstract
A 24 years old female presented with features of severe pulmonary hypertension and right sided heart failure with thrombocytopenia and hematochezia.CT pulmonary angiogram revealed chronic pulmonary thromboembolism with secondary pulmonary hypertension. She also had DVT of both lower limbs.Further workup revealed presence of ANA,AntidsDNA,LAC APL antibody.Pulmonary embolism may be the first manifestation of the APS and recurrent pulmonary emboli may give rise to pulmonary hypertension. So we report this case as a rare initial presentation of SLE as pulmonary HT due to chronic pulmonary thromboembolism with secondary antiphospholipid antibody syndrome
