INTRODUCTION OF INTERNAL CONTROL OF QUALITY AND SAFETY OF MEDICAL ACTIVITIES IN HEALTHCARE ORGANIZATION

The article presents the experience in introduction of the internal system of control of quality and safety of medical activity in a healthcare organization engaged in rendering primary medico-sanitary dental care to the population. The study was conducted in Municipal Dental Outpatient Clinic №3 of Ryazan where the three-level control system was introduced.The internal quality control of medical care was carried out on the basis of a medical card of a dental patient (form 043/у). The authors detected a decline in the share of identified defects in medical care from 8.6% in 2014 to 7.4% in 2016 despite the increased number of expert examination scarried out using time cards of expert evaluations (from 5114 to 5974, respectively). The introduction of the given control system with continuous evaluation into the work of the dental outpatient clinic motivated the medical personnel to increase responsibility for rendering medical care to the population. Here, the awareness of the doctor of the inevitable reduction in the incentive rewards (disciplinary penalties) for each case of rendering low-quality medical care significantly raised responsibility of each medical worker and provided increase in the quality and safety of medical care. Rewards and additional incentive rewards for high-quality work stimulated improvement of medical care quality: in 2016-2017 not a single justified claim was received from patients. The experience of Municipal Dental Outpatient Clinic №3 in use of the internal control system is nowadays being spread on the regional level – it is being introduced into the activity of healthcare organizations with the aim of improvement of the quality and safety of their activity.
