Influence of Risk Factors on the Development and Progress of Psychosomatic Health Disorders at the Level of First Primary Medical Care

Abstract

The article presents the study of risk factors and their influence on the development of psychosomatic health while giving the first primary medical care. The purpose of the study is to investigate the influence of risk factors on the development and progression of psychosomatic health disorders of the population, namely the frequency of stress formation, taking into account the signs of anxiety and depression as the main indicators of existing psychosomatic health disorders, at the level of first aid. The object and methods of research. The study comprised 532 patients (136 males, 396 females) who applied for medical assistance to Bucha City Clinic of Kyiv Oblast. The research was carried out within the framework of the developed program by the method of sociological survey according to the developed questionnaire, which included a certain list of questions. Revealed that 144 (27.1%) of respondents are dissatisfied with low sanitary conditions, accompanied by an increased risk of depression in physical illness to backdrop 2.06 times (p = 0.0001). At the same time, dissatisfaction with working conditions psychological mark of 183 (34.4%), which makes a significant increased risk of depression in this group of patients in the 1.98 times - OR = 1,98 (1,38-2,85). Analysis of self-assessment showed that 148 (27.9%) of respondents define status as "bad", 330 (62.2%) - "medium", 53 (9.9%) - "good". The incidence of depression is associated with poor condition 91 (61.5%), while in good material condition depression is met less frequently - in 19 (35.8%), p = 0,001, OR = 2,86 (1,42-5, 81). The bad material condition, probably, determines the appropriate characteristics of the availability of medicinal products. The availability of drugs at a cost is noted by 157 (29,5%) surveyed. In this category of somatic patients, the incidence of depression is lower - 52 (33.1%). The inaccessibility of drugs at cost is associated with a significantly higher frequency of depression - 200 out of 375 examined (53.3%), p = 0.0001, OR = 2.31 (1.54-3.48). The conducted research allowed drawing the following conclusions: The study defined factors that have a significant impact on the development of the disease. The leading role in the development of psychosomatic health disorders of the population belongs to the medical-social and socio-economic factors. It is important to note the need to pay more attention to psychological health, to carry out a survey of patients more often in order to detect early depression as it is detected in almost half (47,37%) of the subjects. Further study of depression in patients with psychometric scales will significantly increase the effectiveness of therapy due to correlation of their psycho-emotional sphere.
