Abstract

The increase in the incidence of acute cholecystitis in recent years in the elderly is accompanied by high mortality and complications, and this statistics prompts us to search and develop new methods of treatment and rehabilitation of postoperative patients. The aim of our work was to increase the effectiveness of treatment of patients with acute calculous cholecystitis using ozone saline solution. Application of ozone is based on its strong oxidative and sterilization action. Under its influence, not only bacteria die, but also fungi and viruses. The anti-inflammatory effect of ozone is based on its ability to oxidize arachidonic acid and its derivatives - prostaglandins and leukotrienes - biologically active substances that are involved in the development and maintenance of the inflammatory process. In the period from 2011 to 2016, was performed an analysis of surgical treatment of 37 elderly patients with acute cholecystitis. Age of patients from 61 to 89 years. The main group - 20 patients, operated and received with standard therapy with combination of an ozonized saline solution in the postoperative period for the first five days. A comparison group - 17 patients who after a surgical intervention were prescribed standard therapy without ozonized solution In gerontologic patients with acute calculous cholecystitis in the early postoperative period was used ozonated physiological solution, which gave the possibility of a faster restoration of the above-mentioned disorders. In 89,1% of patients of both groups of the elderly with acute calculous cholecystitis there are significant violations of the central hemodynamics by the hypodynamic type with predominance of the isometric mechanism of compensation of violations of cardiovascular functions, which are characterized by decrease in pulsatile volume, minute volume of blood, increase of the general peripheral vascular resistance and violation of hepatorenal perfusion. The application of ozonized saline solution in gerontologic patients with acute calculous cholecystitis in the early postoperative period gives an opportunity for better restoration of blood flow in the liver and impaired cardiovascular functions, which reduced the presence of patients in the hospital by 2 ± 0,3 days.
