ASSESSMENT OF COGNITIVE DYSFUNCTION IN PATIENTS WITH VARIOUS STAGE OF CHRONIC OBSTRUCTIVE PUL-MONARY DISEASE

Abstract
With the purpose of finding the connection between the occurrence of cognitive impairment (CІ) and the level of carbonation of blood oxygen a comparative analysis of the data of indexes in patients with chronic obstructive pulmonary disease (COPD) and practically healthy persons (PHP) was conducted. 120 patients with COPD and 25 PHP aged 49 to 61 participated in the research. Research methods are clinical – analysis of complaints, life and disease history, general and neurological examination, neuropsychological research, instrumental (pulse oximetry), laboratory research, statistical analysis. Patients with COPD II degree of bronchial obstruction had no CІ. The results of neuropsychological studies in patients with COPD III degree of bronchial obstruction indicated that there was presence of mild CІ: the total score on the MMSE scale was significantly different from that of the PHP, and the group I was 26 [25; 27] points (p <0,05), on the scale of МоСА – 24 [24; 25] points (p <0,05), on the scale of FAB – 14 [14; 15] points (p <0,05), the time of filling the tables of Schultz – 254 [251; 257] s (p <0.05). Data of neuropsychological researches indicated that patients with COPD IV degree of bronchial obstruction had the most expressed CІ, which corresponded to moderate cognitive disorders-the initial stage of dementia. On the MMSE scale, the overall score dropped compared to the PHP and the I, II group to 23 [23; 24] points (p <0,05), on the scale of МоСА – 22 [22; 23] points (p <0,05), on the scale of FAB – 12 [11; 12] points (p <0,05), and the time for filling of Schultz tables up to 287 [285.5; 296.5] s (p <0.05). The level of blood oxygen saturation in patients with COPD II degree of bronchial obstruction was 97 [97; 98] % (p <0.05), COPD III degree of bronchial obstruction – 91 [89; 93] % (p <0,05), COPD IV degree of bronchial obstruction – 83 [81; 85] % (p <0.05). Conclusions. It is determined that in these patients the amount of memory and concentration of attention is reduced and significantly different from the PHP. The results of our study have shown that a decrease in the level of carbonation of blood with oxygen may be one of the factors of the occurrence of CІ in patients with COPD: the formation of moderate cognitive impairment-the initial stage of dementia in patients with COPD III-IV degree of bronchial obstruction and the presence of a correlation between the level of cognitive impairment and the level of blood oxygen saturation.
