Abstract

Background. The results of studies in type 2 diabetes mellitus (DM) show changes in the immune system on the cellular, humoral levels. Sustained immunosuppression, caused by hyperglycemia, insulin resistance and adaptive hyperinsulinemia, promotes the development of acute and chronic purulent-inflammatory processes in the body, including soft tissues. The purpose of the study is to study the morphometric and morphofunctional state of peripheral blood leukocytes in patients with type 2 diabetes, to assess their significance in the development of purulent-inflammatory diseases of soft tissues. Materials and methods. Morphometric, chemical and laboratory methods of control (20 healthy volunteers) and the main (47 patients with purulent-inflammatory diseases of soft tissues (PIDST) on the background of DM-2 type) were exam-ined in the work. The groups were studied with statistical pro-cessing of the obtained results. Results Investigation of morpho-functional features of cells of non-specific protection at PIDST against the background of type 2 DM showed that the basis of diabetic complications are detected violations of bactericidal activity of segmental neutrophils. The trend of indicators of leukocyte formulas and dissipation of the distribution of mono-nuclear cells by size, as well as the growth of the lymphocytic granulocytic index, indicate the chronic formation of endotoxins in DM, which creates conditions for the development and pro-gression of PIDST. Conclusions: lowering the level of cationic proteins of neutrophils, myeloperoxidase activity and NST-test under conditions of stimulation of neutrophil granulocytes by lectins is a sign of unsatisfactory state of the bactericidal system of cells, which may be the cause of chronic and acute inflamma-tory processes in the body that accompany type 2 DM, which leads to functional exhaustion of macrophage links of immune protection of the body.
