Successful treatment of Cytomegalo virus retinitis in post renal transplant patient on immunosuppression

Abstract
AimOur aim is to report successful treatment of Cytomegalo virus retinitis (CMV) in post renal transplant patient on immunosuppression and compromised renal status. MethodsDetailed history was followed by slit lamp and indirect ophthalmoscope examination. Routine uveitic work up was carried out following which patient was clinically diagnosed to have CMV retinitis based on her fundus findings. Diagnosis was confirmed by polymerase chain reaction(PCR) test of vitreous aspirate. Renal parameters were assessed. Route of administration of the drug was adjusted accordingly. The extent of necrosis was documented by fundus photographs at the time of presentation and after treatment. Intravitreal ganciclovir was injected. Patient was followed up for one year. Results Retinitis was significant on her first visit. Inflammatory control was achieved by intra vitreal ganciclovir treatment. Patient had no recurrence for one year. Systemic immunosuppression was continued to prevent rejection of renal transplant. Conclusion CMV retinitis is an important opportunistic infection in immunocompromised individuals including renal transplant patients. Immuno suppressives cannot be stopped because of potential renal transplant rejection. Early diagnosis and prompt treatment with intra vitreal Ganciclovir can be effective in full recovery and prevention of vision loss.
