Evaluation of Serum Amylase in the Patients of Obese Type 2 Diabetes Mellitus

Abstract

BACKGROUND. Diabetes Mellitus is a chronic metabolic disorder caused by a derangement in the secretion or function of the endocrine and exocrine portions of the Pancreas due to the close anatomical and functional relationship between its endocrine exocrine portions. So, the current study was designed to evaluate the Fasting Plasma Glucose and Plasma Amylase levels of diabetic patients as representatives of the two portions of the Pancreas respectively. AIMS To determine the Fasting Plasma Glucose, serum Lipid Profile and serum Amylase in known cases of Type 2 Diabetes Mellitus and to compare Serum Amylase levels with the duration of Diabetes Mellitus. MATERIALS AND METHODS50 cases of Type2 Diabetes Mellitus with BMI 30Kgm2and 50 healthy non obese age sex matched controls were chosen. Fasting Plasma Glucose, Serum TGL and Total cholesterol were measured by enzymatic methods. Serum HDL was measured by Phospho tungstic acid method. Serum VLDL LDL were calculated by Friedwalds formula. Serum Amylase was estimated using a chromogenic substrate CNPG3.RESULTSIn this study, Mean Serum Amylase level among cases (28.3 IUL) was lower than that of control group (74.90 IUL). Mean Amylase level of 5yrs duration,5-10 yrs duration and 10yrs duration of diabetes were 37.88 IUL, 27.36 IUL and 20.62 IUL respectively. so,as the duration of diabetes increases amylase level decreases. Mean Fasting Plasma Glucose levels of cases (146.82 mgsdL) was higher than that of controls (91.62mgsdL).MeanTotalCholesterol (167.54mgsdL),Triglycerides (155.32mgsdL),LDL (102.49mgsdL),VLDL(31.06mgsdL),Total cholesterol HDL ratio (5.08) of cases were higher than that of controls, whose MeanTotalCholesterol(145.44mgsdL),Triglycerides (115.12mgsdL),LDL(74.67mgsdL),VLDL(23.02mgsdL) Total cholesterolHDL ratio (3.45).Mean Serum HDL level of cases ( 3 3 . 9 8 m g s d L ) w a s l o w e r t h a t o f c o n t r o l s (42.9mgsdL).CONCLUSIONThis study demonstrated that low Serum Amylase levels associated with Type 2 diabetes patients suggesting a possible exocrine-endocrine relationship in this disease.
