VALUE OF CHANGES OF THE CURTAILING SYSTEM OF BLOOD IN PATHOGENESIS OF DEVELOPMENT OF PLANOCELLULAR CANCER OF SKIN

Abstract
Aim. To study changes of the curtailing system of blood in pathogenesis of development of planocellular cancer of skin. Materials and Methods. The condition of the curtailing system of blood was studied at 39 patients with planocellular cancer of skin whose age was 55±3 years. Researches were conducted before expeditious treatment, in the first postoperative day, for the fifth, seventh, tenth postoperative day and in 18 months after the undergone operation. For comparison indicators of the curtailing system of blood at 17 rather healthy volunteers of the same age and a floor (group of comparison 1) and 28 patients, operated skin fibromas are studied (group of comparison 2). Results. As a result of the conducted research it is established that at planocellular cellular skin cancer prior to expeditious treatment, dysfunction of a vascular wall an endoteliya was also noted that is confirmed by increase in an endotelin. Increase in an indicator of an endotelin happened in 6.1 in comparison with the data obtained in group of rather healthy people. It also led to development of a powerful vazokonstriktion and strengthening of prokogulyatny activity of blood. In this group of patients, there was a prolonged increase in prokogulyatny activity of blood, along with it there was an oppression of the anticurtailing and fibrinolytic activity of blood that is confirmed by increase in activity of AT III and vWF and simultaneous decrease in the tPA-PAI-1 complex. All this indicates that it endoteliya of vessels low produces AT III and fabric the activator of a plazminogen of tPA which in 90% contains in blood in the form of the tPA-PAI-1 complex at planocellular cancer of skin. It is established that in pathogenesis of development of planocellular cancer of skin microcirculation violation lies. At the same time in pathogenesis of development of fibroma of skin, these violations also happen, however, at fibroma of skin of change in microcirculation are less expressed, microcirculation is restored for the third postoperative day. Performance of expeditious treatment at planocellular cancer of skin, is not led to correction of violations of microcirculation. Conclusion. In pathogenesis of development of planocellular cancer of skin violation of synthesis of factors of antifolding of a fibrinoliz and atikoagulyant lies that leads to strengthening of prokogulyatny activity of blood.
