Abstract
Unilateral proptosis with raised intraocular pressure is a very rare clinical presentation. We hereby report a 34 year old healthy male patient with unilateral proptosis with raised intraocular pressure and dilated episcleral vein in one eye. We suspected carotico-cavernous fistula and did ultrasound. Ultrasound revealed dilated superior ophthalmic vein suggestive of this condition. After referring to an interventional radiologist , MRI showed dilated superior ophthalmic vein and MR angiography showed direct carotid-cavernous fistula. This pathology can be treated successfully in 90 percent of all cases.
