Giant broad ligament leiomyomas are rare in postmenopausal women. However, we had one case of giant broad ligament leiomyoma with third degree UV prolapse. This case is reported for its rarity and diagnostic difficulty. A 51 years old post menopausal lady with mass abdomen and third degree UV prolapse was evaluated and diagnosed as ovarian mass. Exploratory Laparotomy was done, which showed right broad ligament fibroid. TAH with BSO done with excision of right broad ligament fibroid. Since she is a case of broad ligament leiomyoma with minimal cystic degeneration presenting at post menopausal age with third degree UV prolapse probably due to pressure effect is a rare case presentation.
