Prognostic Technologies and their Implementation in a Patient-Oriented Strategy for Chronic Dermatoses Patients Management: Innovative Decision of Typical Tasks in the Activity of a General Practitioner of Family Practitioner Medicine

Abstract

The purpose of the study was to systematize the author's innovative developments regarding the introduction of prognostic models in the professional activity of the general practitioner of family medicine in the integrated management of patients with chronical dermatosis. Material and methods. The original material of analysis and structural-logical generalization was scientifically substantiated and published in 2008-2018 by author's innovations in prognosis and assessment of the quality of health, methodology for determining the prophylactic potential of a complex of social and medical, regional-ecological, constitutional-biological and family-genealogical risk factors of chronical dermatosis. The systematization of innovations was performed in accordance with the Typical Activity Tasks of a general practitioner of family medicine, provided by the educational standards of Ukraine, the standard curriculum of internship and the qualification of the general practitioner of family medicine, as well as the passport of scientific specialty of general practitioner of family medicine. They are as follows: the results of the researches covered in the monographs, are set forth in the patents of Ukraine for inventions and utility models, branch innovations of the Ministry of Health of Ukraine to the system of providing medical assistance in cases of chronical dermatosis, information bulletin of area standards and the methodological recommendations of the Ministry of Health of Ukraine, as well as algorithms and technologies for forecasting / estimating the risk oriented to the primary level of medical assistance in chronical dermatosis. These problems were presented in the author's works (independently and in co-authorship). The methods of system approach and problem-target analysis, bibliosematy and congruent modeling of functional and structural models of the activity of a general practitioner of family medicine are used in the work. Results and discussion. The research carried out a problem-oriented analysis of author's prognostic technologies, which solve the typical tasks of a general practitioner of family medicine and, on the basis of structural and functional analysis of the educational-professional program at the pre- and post-graduate stages of preparation, as well as qualification characteristics, the competences of a general practitioner of family medicine for patients with chronical dermatosis. It was proved that the innovative methodological solving of the typical tasks of a general practitioner of family medicine allowed to implement a patient-oriented approach. Moreover, it was congruent (functionally appropriate) with the basic directions of educational training and technologies of practice of a general practitioner of family medicine, including interaction and interconnection (team collaboration) with specialists of higher levels of medical care provision. Conclusions. It was proved that in the sphere of a general practitioner of family medicine competence was the personalization of the level of chronical dermatosis risk according to regional-ecological, biological and medical-social determinants, distribution to the groups of primary and secondary prevention with the assessment of the quality of health and risk of the formation of comorbid dysplastic-dependent pathology in patients with chronical dermatosis. Varieties of innovative prognostic technologies, risk assessment algorithms and examples of their use at the primary level were analyzed, and the aspects of coordination of cooperation with II-III levels of medical care were also identified.
