Comparative Assessment of Changes in Biochemical Indices of Oral Liquid in Patients with Chronic Odontogeny Sinusitis during the Standard Treatment with Electroacupuncture

Abstract
From 5% to 15% of adults and 5% of children suffer from sinusitis. One third of this pathology is represented by its odontogenic forms. Epidemiological studies in more than 30 countries indicate a 3-fold increase in the incidence of sinusitis over the past decade. There is an increase in the frequency of this pathology by 1.5-2% annually. Electroacupuncture has been repeatedly proved to have an effective therapeutic and rehabilitation effect which is successfully used in patients with surgical pathology of the maxillofacial region in the postoperative period including infectious and inflammatory diseases of the maxillofacial region and neck. However, in the special literature there is no information about the possibility of correction of biochemical parameters of the oral fluid of patients with chronic odontogenic sinusitis of the maxillary sinus with the help of electroacupuncture. The purpose of the work was to evaluate the effectiveness of the standard treatment and the use of electroacupuncture in its composition on the basis of comparison of the dynamics of biochemical indices in oral fluid of patients with chronic odontogenic sinusitis. Material and methods. We examined 40 patients aged 18-50 years with a diagnosis of chronic odontogenic sinusitis of the maxillary sinus. The diagnosis of chronic odontogenic sinusitis of the maxillary sinus with limited lesion was localized in the area of the bottom of the mentioned sinus, and verified by cone-beam computed tomography. Group I (20 patients) after surgery had the standard treatment. Group 2 (20 patients) included electroacupuncture in the treatment. The acupuncture points were affected with the help of the DENS device in the mode of "Therapy" at a frequency of 77 Hz and a comfortable level of power for the patient. Group 3 consisted of 10 healthy individuals. Determination of the activity level of alkaline phosphatase (AP), calcium ions (Ca2+), phosphorus ions (P) was performed in dynamics: before the operation, on the 1st, 7-th and 12-th days after the intervention. Biochemical indices were determined with an automatic biochemical analyzer (Random Access A-25, Spain) and a standard set of reagents. The obtained data were statistically processed using the "Statistisa 10.0" application package. Results and discussion. At the end of treatment, the group 1 indices retained significant differences with the norm: for AP ˗ H=11.67 (p=0.003), z1-3=3.35 (p=0.003); for Ca2+ ˗ H=9.0 (p=0.01), z1-3=2.90 (p=0.01); for P ˗ H=8.8 (p=0.02), z1-3=2.52 (p=0.04). Indicators of the group 2 such significant differences were identified: for AP ˗ H=11.67 (p=0.003), z1-3=of 1.19 (p=0.70); for Ca2+ ˗ H=9.0 (p=0.01), z1-3=0.82 (p=1.00); for P ˗ H=8.8 (p=0.02), z1-3=0.15 (p=1.00). Conclusion. The presented results of comparative assessment of the dynamics of biochemical indices of oral fluid in patients with chronic odontogenic sinusitis proved the advantage of a group of people who were treated with electroacupuncture and convinced the effectiveness of its use in this category of patients. Based on the principles of evidence-based medicine, the positive results of electroacupuncture in the complex postoperative treatment of patients with chronic odontogenic sinusitis allow us to recommend this method for wider implementation in practical health care which will reduce the number of complications, reduce the overall duration of treatment, improve the quality of life of patients and the level of specialized medical care.
