Miadında Doğumda Serebral Palsi’ye Yol Açan İntrapartum Risk Faktörlerine Yaklaşımın Yeniden Değerlendirmesi

Cerebral palsy (CP) is a symptom complex, particularly of movement and posture disorders, which are often accompanied by musculoskeletal and neurological problems. The incidence of CP in term babies varies between 1.0 and 1.7 per 1000 live births. There are more medical malpractice lawsuits in Turkey in the field of obstetrics and gynecology (16.82%) than in any other field of medicine. Almost all of these obstetrical and gynecological cases (96%) are complications that occur during deliveries. In fact, most other countries in the world have similarly high rates of medical malpractice lawsuits in obstetrics and gynecology. This is the main reason why in recent years the number of studies and discussions aiming to determine whether the damage to the child's central nervous system has occurred before, during, or after the delivery have increased in number. There is a noticeable tendency in research into factors leading to CP to overemphasize the importance of prenatal factors, which, if valid, would diminish the responsibility of the obstetrician. With all these factors in mind, we reviewed recent literature on obstetrical management's contribution to CP, and have reached the conclusion that current intrapartum management guidelines must be reviewed and new paradigms sought. Bu nedenle son yıllarda bu davalara konu olan ve Serebral Palsi (SP) ile sonuçlanmış vakalarda çocuğun merkezi sinir sisteminde oluşan hasarın doğumdan önce mi, doğum esnasında mı ya da doğum sonrasında mı gerçekleştiğini saptama amacını güden araştırma ve tartışmaların sayısı çoğalmıştır. Bu araştırmalar sonunda SP'ye yol açan nöronal hasarın özellikle doğum sırasında gerçekleşenleri konusunda edinilmiş olan bilgiler, halen yürürlükte olan birçok klasik doğum yaptırma yönteminin gözden geçirilmesi zamanının geldiğini düşündürmektedir. Biz bu konudaki literatürü tarayarak doğum yaptırma stratejilerinde izlenmesi gereken yeni paradigmaları belirlemek istedik.
