TROPHIC ULCERS OF VENOUS GENESIS: MODERN TREATMENT POSSIBILITIES

Abstract

25-30 % of adult population in Belarus suffers from varix dilatation, and 15-18% of them develop chronic venous insufficiency. More than 40 thousand patients suffer from venous trophic ulcers of the lower extremities. High prevalence of trophic ulcers representing a threat to many aspects of patients’ quality of life, absence of the tendency towards the decrease in the disease incidence, long and persistent course, liability to relapses, permanent restriction of capacity for work leading to physical disability in 12.5 % of patients, is not only a medical, but also social and economic problem. Search of possible ways to improve treatment results of venous trophic ulcers remains actual. The purpose of the work was to analyze efficiency of complex conservative, surgical and local treatment of trophic ulcers of venous genesis accepted in our clinic. Our experience of treatment of 847 patients with trophic ulcers of the lower extremities of venous genesis is presented. We used the following methods: medicamentous therapy, stage necrectomies and opening of purulent leakage in 258 (30.4%) patients, phlebectomy in 106 (12.5%), autologous grafting of ulcers in 29 (3.4%) patients. The offered composite materials and photodynamic therapy were applied locally in all patients. Healing of ulcers was observed in 510 (60.1%) patients, in 337 (39.8%) patients complete healing of ulcers was not obtained. Relapse of ulcers and progressing of purulent process were noted in 117 (22.9%) patients in 2-4 months after treatment.
