Adult onset Bartter's syndrome- a case report

Abstract

Bartters syndrome is a hereditary disorder causing metabolic alkalosis and hypokalemia without hypertension. They are due to genetic mutation involving any of four ion transport proteins in the thick ascending loop of Henle in the kidney. Our patient presented with pure motor paraparesis, investigated and found to e due to hypokalemia. On further investigation for the cause of hypokalemia, patient was found to have metabolic alkalosis, hypercalciuria, normal blood pressure and normal magnesium. Based on the above features , a diagnosis of Bartters syndrome was made. This case was presented since the onset was in adult age group and patient had no complications of the disease so far.
