A RARE INTERESTING CASE OF SPLENIC PSEUDOCYST

Abstract

A 16 years old girl presented with complaints of left upper abdominal pain of 2 years duration. Pain was also radiating to back. Pain was aggravated on lying supine and after food intake and was relieved on leaning forward. The patient also had constitutional symptoms like nausea, vomiting, fever, loss of appetite and weight. The patient also gave a history of jaundice 6 months. Examination of abdomen revealed a normal abdomen except for mild tenderness in the left hypochondrium and epigastrium. Ultrasound of abdomen showed a well defined 7 x 8 cm hypoechoic lesion in the spleen with internal echoes and solid components and the picture was suggestive of a splenic pseudocyst or abscess. Contrast CT of abdomen was done which confirmed the diagnosis as acute on chronic pancreatitis with an intrasplenic pseudocyst. Patient was taken up for laparotomy after giving penumococcal vaccine and splenectomy was done. Post operative histopathology confirmed our preoperative diagnosis of splenic pseudocyst.
