FORMATION OF CREATIVE CLINICAL THINKING OF STUDENTS ON THE BASIS OF THE EUROPEAN CREDIT TRANSFER SYSTEM OF EDUCATIONAL PROCESS ON THE RESULTS OF UKRAINIAN STUDENT COMPETITION IN THERAPY (INTERNAL MEDICINE)

Abstract

Introduction. In modern medicine therapy as before remains a fundamental integrated discipline that summarizes clinical problems of internal diseases and focuses on general changes in the patient. Therapy based on the achievements of natural sciences and philosophy, considers the diseases of internal organs as the only pathophysiological process of the body and offers a wide arsenal of preventive, curative and rehabilitative interventions. Therefore, the development of clinical thinking of future specialists is one of the priorities of medical education required by the European credit transfer system of educational process (ECTSEP). The aim of the study. To analyze the results of the Ukrainian student competition in therapy (internal medicine) in 2016 and to establish predictors of the clinical forming of creative thinking of students on the basis of theoretical knowledge and practical skills in accordance with requirements ECTSEP. Materials and Methods. The criteria were defined for assessing the level of practical, clinical training of participants. Stage I of the competition - test control of knowledge on content modules (cardiology, nephrology, rheumatology, allergology, gastroenterology, hematology). Stage II of the competition - work in conditions close to professional activity - "at the bedside of the therapeutic patient" (hematology, rheumatology, gastroenterology, allergy, nephrological department Regional Hospital). The third stage - evaluation of clinical thinking by the model of solving situational problems in diagnostics, tactics of conduction and emergency for the therapeutic patient. Results. The Contest demonstrated a high level of students preparation in therapy (internal medicine). The highest percentage of correct answers to test questions in stage I was 100%, the lowest - 59%. Most of the students gave correct answeres to 60-83% of questions. Results of supervision of patient during the second stage: the best result - 150 points (16.21%), the lowest - 70-95 points (10.81%), the majority of students gained 105-145 points (72.97%). At the stage III of competition the best result was 98.6 points, and the lowest - 52.9 points. Assessment of practical skills of clinical examination of the patient (Stage II) showed a sufficient practical training of students as for the appointment and interpretation of necessary laboratory and instrumental studies, diagnosis according to the requirements of the modern classification of internal diseases, taking into account the period, form of the disease, as well as the appointment of treatment for the individual patient. Solving clinical problems of high complexity (third stage) showed that medical students know well nosological and differential diagnosis and treatment of certain cardiovascular diseases and emergency conditions, such as acute coronary syndrome, myocarditis, cardiomyopathy and certain arrhythmias and syndromes (atrio-ventricular nodal tachycardia, extended QT interval syndrome, Wolff-Parkinson-White syndrome, Frederick syndrome Morgani-Adams-Stokes syndrome). Conclusions Ukrainian student olympiad in therapy (internal diseases) is a modern technology of development and creative clinical thinking of students as an integral part of the educational process in accordance with requirements ECTSEP. Application of modern computer technology and interactive methods of visualization (electrocardiograms, photo and/or video protocols, echocardiography, coronary angiography, multislice computed tomography, MRI, etc.) allow to evaluate and develop clinical thinking among students during a nationwide student competition in therapy (internal diseases).
