СONSTRICTION OF THE THICK COLON IN THE RIGHT BOGDALEK`S TRIANGLE

Abstract

The causes of the constriction development are congenital defects or weak areas, large sizes of natural apertures of the diaphragm. Congenital defects are more often observed on the left in the posterior division of the diaphragm at the lumbar-costal hernia (Bogdalek`s hernia). However, the presence under the right dome of the diaphragm of a large lobe of the liver, practically, makes it impossible to develop Bogdalek`s hernia through the lumbar aperture triangle of the diaphragm to the right. In 70-80% of cases, the Bogdalek`s hernia is located on the left side, respectively, in 20-30% of cases - on the right. Hernias in the front region of the right diaphragm-mediastinal sinus and parasternal hernia (Loreya) are quite rare. At the same time, the hernia of the esophageal aperture of the diaphragm (HEAD) is a widespread disease and occurs in up to 95% of all diaphragmatic hernias. Congenital defects are more often observed on the left in the posterior division of the diaphragm with the output of the lumbar-costal hernia (Bogdalek`s). Sometimes, even with obvious signs of AIO (acute intestinal obstruction), their symptomatology are interpreted as a pathology of other organs. Objective. Demonstration of a rare case of internal latching of the loop of the thick colon and of the cape gut in the right back side division of the diaphragm at the lumbar-costal Bogdalek`s hernia. Discussion. The clinical case of successful diagnosis and surgical treatment of internal hepatic latching in the right Bogdalek`s triangle of the diaphragm, which was manifested by low intestinal obstruction, and simulated the radiological picture of the oncopathology of the lung, or of the lower-lobe right side abscess pneumonia. Conclusion. The given clinical case of jamming of the loop of the colon in the right Bogdalek`s triangle, caused by excessive physical exertion and the development of acute intestinal obstruction with the simulation of the picture of the lower-lobe abscess pneumonia, is rare and therefore has a scientific and practical value for general and abdominal surgeons, with differential diagnosis and timely operative treatment.
