Abstract

ABSTRACT Malignant transformation of ovarian teratoma is an infrequent 1, often asymptomatic event most commonly in the postmenopausal age of which squamous cell carcinoma accounts for 80 of the transformation because of the high density of ectoderm in these tissues2. It has high concentrations of squamous cell carcinoma SCC antigen and Ca 125 levels, with ovarian tutor more than 100mm3 in size. Incidence1 being 0.17 1.3 of all teratomas. Here we report a 39 yr old menstruating lady who presented with vague lower abdominal pain , fever and loss of weight evaluated clinically and radiologically to be a suspected case of malignant ovarian dermoid. Staging laparotomy done and proceeded to TAH with BSO. Histopathologically confirmed. Staged as stage 2c. Postoperatively advised 3 cycles chemotherapy and 3 monthly follow up for two years by medical oncologist. Hence we conclude that though the secondary development of malignant transformation is rare it is a well known phenomenon in patients with ovarian teratoma3 and should be suspected in large dermoid2 and in older patients so that the best can be done to the patient regarding the staging of the disease and its further management.
