METHOD OF RADICAL CYSTECTOMY WITH PRESERVATION OF PROSTATIC CAPSULE WITH ORTHOTOPIC PLASTY OF URINARY BLADDER IN TREATMENT FOR BLADDER CANCER

Abstract
Bladder cancer (BC) ranks 13th in the structure of oncological morbidity in the Russian Federation and accounts for about 2.7% of cases. In 90% of cases BC arises from transitional epithelium of the urinary tract and may take superficial, or non-muscle invasive form with a relatively favorable prognosis, and muscle invasive form characterized by a rapid growth through the muscle layer of the bladder wall and by early metastasizing, which, as a rule, requires radical cystectomy. The main problem that requires solution after such intervention is that of choice of the method of urine diversion. Success of reconstructive operations is first of all determined by the extent of urine continence and a possibility to have an active sex life with achievement of full social adaptation by patients. In the article a clinical example of successful treatment of BC with use of radical functionally sparing cystectomy with preservation of the prostatic capsule and orthotopic plasty of the bladder is described. This intervention permits creation of a spherical bladder of low pressure, preservation of sphincter function of urethra including prostatic capsule thus reducing frequency of urination, it does not impair erectile function and provides urine continence.
