INTERRELATIONS BETWEEN TOTAL AND ALCOHOL-ATTRIBUTABLE MORTALITY AND ALCOHOL CONSUMPTION

Abstract

Background. Significant interregional differences in parameters of total and alcohol-attributable mortality of the population determine the importance of research in the constituent entities of Russia for establishing priority directions for its reduction. Aim. To study the pattern of changes in total and alcohol-attributable mortality in the Republic of Mari El (RME) in 2000-2017 with establishment of significance and extent of dependence on the sale and use of alcoholic beverages. Material and Methods. The analysis includes statistical data of Russia and the Republic. The assessment of the dynamics of the phenomena was carried out by the magnitude of the coefficient of determination. The median, Spearmans correlation coefficient between alcohol parameters and mortality of the population with 95% confidence interval (CI) and a lag of one or more years were calculated. Results. The orientation and the level of the total mortality of the population of the RME in 2000-2017 coincided with the averages parameters for Russia, with a steady downward pattern only in the rural areas. Mortality from alcohol-related causes was higher in the Republic and in the rural areas exceeded that in urban areas. The structure and levels of alcohol-attributable mortality have changed significantly: the proportion of deaths from accidental alcohol intoxication decreased from 93.9 to 57.2%. The links of both types of deaths with the total volume of sold alcohol and the volume per capita turned out to be insignificant. The correlation was revealed with total sales of vodka (r=0.66; p=0.001 and r=0.62; p=0.003). The connection of very high significance and degree was found between alcohol consumption (taken into account + illegal) and the total mortality rate of the population (r=0.94; p=0.000...1) lasting 5 years. The relationship of alcohol mortality with illegal alcohol was lower in magnitude, significance, and duration. Conclusion. One of the leading ways to reduce the total and alcohol-attributable mortality of the population of the Republic was determined to be reduction of the consumption of vodka products and illegal alcohol.
