Constituents of primary healthcare quality as objects of public administration

Abstract

The state grants the right to declarants (patients) to receive primary healthcare (hereinafter referred to as PH) free of charge. What matters here is not so much the provision of PH as such but rather its acceptable quality. Existing methods of assessing the quality of PH involve the use of official statistical data, the results of sociological research and/or the work of the scientific community. At the level of PH facilities, checklists are used (focusing on the quality of a particular instance of PH), whereas, at the territorial level, generalised indicators reflecting the current state and trends in the studied parameters are utilised. Assessment of PH quality is partly carried out in the chain "patient – doctor – PH facility", and therefore involves assessment of the social and medical effect of PH at the level of an individual provider. Importantly, however, the quality of PH depends on a much larger "circle of players" that differ in the level, form, method, and means of influence. Taking into account the results of our own research and the work of leading scientists on this issue, we have improved the list of constituents of the quality of PH: 1) social processes as an object of public administration; 2) the structure of ensuring the provision of PH to the population as an object of public administration; 3) the process of providing PH, which is a combination of public administration and administrative management; 4) behaviour (actions/inactions) of the recipients and providers of PH, which is a combination of elements of administrative management and self-management; 5) the results of PH provision (health, social and economic effects) reflect the effectiveness of public administration, administrative management, and self-management in this area. Further research should be devoted to the development of theoretical provisions and practical recommendations aimed at improving public administration of PH provision to the population
