RESULTS OF COMPLEX IMMUNOTROPIC TREATMENT OF PATIENTS WITH GENERALIZED PERIODONTITIS WITH ACCOMPANYING ISCHEMIC HEART DISEASE, USING MEDICINE OF HERBAL ORIGIN

Abstract

The urgency of the study is that generalized periodontitis as a polyethiological disease with multi-level and ambiguous development mechanisms is a significant healthcare problem. It gives a widespread prevalence of partial or complete complications that negatively affects or prevents chewing, creates aesthetic discomfort, and in future gives a defect, and therefore social maladaptation and quality of life. In Ukraine, the spread of the ailment reaches 80%. Compared with other dental diseases, lesions of periodontal tissues are the result of complicated and profound violations of many biochemical, immunological and endocrinological reactions. Untimely diagnosis of the concomitant disease or pathological condition, which caused structural changes in the periodontal tissues, leads to irreversible processes in them and, as a consequence, to premature loss of teeth. At present, relations of periodontitis with various diseases of the internal organs have been established: cardiovascular ailments, systemic osteoporosis, chronic infectious diseases, metabolic syndrome, type 2 diabetes mellitus, chronic kidney disease (CKD), stroke, malignant tumors, rheumatoid arthritis etc. Current data indicate a close correlation between periodontitis and cardiovascular diseases. A meta-analysis of a number of studies showed an increase of 19% of absolute risk and a 44% relative risk of cardio-vascular disease (CVS) in patients with periodontitis. Among them, CHD, stroke and atherosclerosis of periph-eral arteries dominate. Questions of the probable association of perio-dontal diseases and CHD are actively debated, at least for the past 20 years. Both ailments are multifactorial inflammation conditions with many common risk factors (smoking, type 2 diabetes, obesity, etc.), and the key to their progression and development is the comprehensive effects of inflammation. Quite often, it is problematic to achieve a long-term, qualitative remission of the disease: the complete elimination of clinical signs of inflammation in the perio-dontal disease and the long-term stabilization of bone resorption. Therefore, the traditional approach to general treatment of GP requires optimization. In view of the above, the purpose of our work was to increase the effec-tiveness of treatment of GP in patients with coronary heart disease, using integrated immunotherapy. To achieve the goal, the following tasks have been formulated: to study the effect of the proposed immunotropic therapy of GP in patients with coronary heart disease on indicators of periodontal status. In order to achieve the tasks, the survey methods have been used: the determination of the indices of inflammation PMA, bleeding PBI, hygiene the HI, the depth of the periodon-tal pockets (PP), and the periodontal index of Ramfjord PDI. The proposed treatment scheme included a traditional approach and, in addition, a per os intake of “Immuno-ton” medicine, as well as instillation in the periodontal pockets of the extemporal gel based on the phyto concentrate “Dzherelo” and “Enterosgel”. The obtained results indicate that the application of the proposed treatment method has a veraciously more significant effect on the course of GP and improvement of the state of periodontal tissues compared with the traditional approach.
