PREDICTORS OF ESSENTIAL HYPER-TENSION AND CHRONIC HEART FAIL-URE IN MEN, RESIDENTS OF PODILLYA REGION

Abstract

The research work studied the use of possible predictors in the development of essential hypertension and chronic heart failure. Objective: to improve the prediction of the devel-opment of essential hypertension and resultant chronic heart failure in men of Podillya region of Ukraine, the carriers of polymorphic variants of angiotensin II type 1 receptor gene. The results of instrumental, biochemical and genetic exami-nation (determination of polymorphism of angiotensin II type 1 receptor gene) of 40-60 year old males, the residents of Podillya region of Ukraine were analyzed. There were patients without cardiovascular pathology (79 individuals) and patients with essential hypertensive disease and left ventricular hypertrophy with no signs of chronic heart failure (n = 62). All patients were treated at Military Medical Clinical Center of the Central Region and Vinnytsia Regional Specialized Clinical Hospital of Radiation Protection of Ministry of Health of Ukraine in the period from December 2013 to June 2015. In the course of study, using a multiple step-by-step analysis, it was found that the list of predictors of the development of essential hypertensive disease in indi-viduals without pre-existing cardiovascular pathology includes carrier status of C allele of angiotensin II type 1 receptor gene, body mass index > 30 kg / m2, left atrium size > 40 mm, relative left ventricular wall thickness > 0.42, diastolic dysfunction, left ventricular mass index ≥ 93.57 g / m2, loaded heredity, as well as presence of whorl on the 2nd finger of the left hand, levels of galectin-3 and brain natriuretic peptide. Predictors of chronic heart failure in essential hy-pertensive disease, in addition to the above mentioned indica-tors, proved to be pronounced left ventricular (LV) hypertro-phy, eccentric left ventricular hypertrophy, smoking, onset of essential hypertensive disease before the age of 40, ejection fraction (EF) < 40%, 2 and 3 degree of increase in blood pressure (BP). Significance of brain natriuretic peptide level in blood as a biomarker for predicting the development of chronic heart failure in patients with essential hypertension has decreased, while that of galectin-3 level, on the contrary, has increased as compared to the weight of those predictors in relation to the development of essential hypertensive disease.
