Abstract

Introduction: Acute transverse myelitis (ATM) is a condition characterized by motor, sensory, and autonomic dysfunction due to demyelination and neuronal damage caused by focal inflammation in the spinal cord. This study presents a case of Acute Transverse myelitis accompanied by syringomyelia, drawing attention to the rarity of the disease and its complications. Case Report: A 42-year-old female patient presented with neurological symptoms and the center was thought to be based on contrast-enhanced cranial and cervical magnetic resonance imaging (MRI). MRI showed a cavity with a syringohydromyelia at the widest part of the C2-C5, measured at 5 * 8 mm, without constructive involvement. The patient was followed up in the room air at the neurology service, and lumbar puncture (LP) was performed for diagnostic purposes. The patient's oxygenation deteriorated immediately after the puncture and respiratory arrest developed. The patient was referred to us with orotracheal intubation. Cerebral herniation due to ATM and LP was considered because the pupils of the patient who was taken to our Intensive Care Unit were unizochoric and their respiration deteriorated rapidly after LP. The patient showed improvement in her symptoms and was extubated on the 8th day of her admission. On the 13th day of his admission, she was drained into the neurology ward. Discussion-Conclusion: Although demyelinizing diseases may be malignant, they can recover without sequelae with appropriate support and treatments for etiology and increase the responsibility of physicians in diagnosis and treatment stages. In cervical MRI, it should be kept in mind that syringomyelia with cystic cavity may cause intracranial pressure increase and herniation by cystic cavity in patients with syringohidromyelia.
