COMPARATIVE EVALUATION AND ANALYSIS OF PATHOMORPHOLOGICAL AND ENDOSCOPIC PICTURE OF THE GASTRIC MUCOSA WITH INTESTINAL METAPLASIA

Abstract
A comparative endoscopic and pathomorphological analysis of macroscopic and microscopic changes of the gastric mucosa of 68 patients with chronic atrophic gastritis and intestinal metaplasia was performed. The reliable advantage (t = 7,44, p < 0,001) of chromoendoscopy over common endoscopy in diagnostics of intestinal metaplasia was revealed. We marked out 3 main types of intestinal metaplasia according to macroscopic changes of gastric mucosa: focal, focal and confluent and diffuse (multifocal). Depending on localization and extension of intestinal metaplasia: 1) antral (predominantly around the pylorus); 2) subtotal (with extension along the lesser curvature up to the upper third of the corpus); 3) total (with extension along the lesser curvature and greater curvature and consecutive involving in process of the fundic and cardiac parts of the stomach. According to the involved area: slight extent ? involving <20 % of gastric mucosa area; moderate ? 20-50 %; severe extent ? >50%. The use of chromoendoscopy as screening test in patients with intestinal metaplasia of a gastric mucosa was proposed.
