COGNITIVE DYSFUNCTION ASSESSMENT IN PATIENTS WITH VARIOUS STAGES OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Abstract

Objective. With the purpose of findingthe connection between the occurrence of cognitive impairment (CІ) and the level of carbonation of bloodoxygen, a comparative analysis of the data of indexesinpatients with chronic obstructive pulmonary disease (COPD) and healthy people (PHP) was conducted. Materials. 120 patients with COPD and 25 PHP aged 49-61 were investigated in the research. Research methods are clinical analysis of complaints, life and dis-ease history, general and neurological examination, neu-ropsychological research, instrumental (pulseoximetry), laboratory research, statistic alanalysis. Results. Patients with COPD degree II of bron-chial obstruction had no CІ. The results of neuropsycho-logical studies in patients with COPD degree III of bron-chial obstruction in dicated that there wasa presence of mild CІ: the total score on the MMSE scale was signifi-cantly different from that of the PHP, and the group I was 26 [25; 27] points (p <0,05), on the scale of МоСА – 24 [24; 25] points (p <0,05), on the scale of FAB – 14 [14; 15] points (p <0,05), the time of filling the tables ofSchultz – 254 [251; 257] s (p <0.05). Data of neuropsy-chological research indicated that patients with COPD degree IV of bronchial obstruction had the most ex-pressed CІ, which corresponded to moderate cognitive disorders-the initial stage of dementia. Onthe MMSE scale, the overall score dropped compared to the PHP and the groups I, II to 23 [23; 24] points (p <0,05), on the scale of МоСА – 22 [22; 23] points (p <0,05), on the scale of FAB – 12 [11; 12] points (p <0,05), and the timefor filling of Schultz tables upto 287 [285.5; 296.5] s (p <0.05). The level of blood oxygen saturation in pa-tients with COPD degree II of bronchial obstruction was 97 [97; 98] % (p <0.05), COPD degree III of bronchial obstruction – 91 [89; 93] % (p <0,05), COPD IV degree of bronchial obstruction – 83 [81; 85] % (p <0.05). Conclusions. It is determined that in patients with COPD the level memory and concentration of at tenti on is reduced and significantly different from the PHP. The results of our study have shown that a decrease in level of carbonation of blood with oxygen maybe one of the factors of the occurrence of CІ in patients with COPD: the form at ionofmoderate cognitive impairment-the initial stage of dementia in patients with COPD degree III-IV of bronchial obstruction and the presence of acorrelation between the level of cognitive impairment and the level of blood oxygensaturation.
